‘Patient Safety First Declaration’ – Working together for quality services

(Clearly not all commitments are equally relevant for all signatories, but all signatories adhere to the principles contained herein.)
In signing up to the Patient Safety First initiative and in using the logo on all our communications:
1. I am committing my organisation to accord the highest level of priority to the safety of patients and the quality of care that they receive
2. I am committing my organisation to be an active participant in the implementation of the patient safety initiatives particularly those that are part of the implementation of the report of the Commission on Patient Safety and Quality Assurance
3. I am committing my organisation as appropriate to be an active participant in specific initiatives in:
· medication safety
· the education of staff
· the governance and accountability of our health services
· the adoption and implementation of quality standards
· the credentialing of staff working in the health services
· improvements in the regulation of health professionals
· the systematic adoption of audit of the health services that we provide
· the adoption and implementation of all nationally agreed clinical guidelines
· the open reporting of all incidents or adverse events that may affect patients in a harmful fashion
· the involvement of service users in decision making affecting their care and affecting the services that they access.
